MENASHA, Wis. - Dr. Andrew Weiss, OB/GYN, will now be seeing patients at the Affinity Medical Home in Kaukauna (2601 Fieldcrest Drive) on Wednesdays. Weiss’ focus on obstetrics and gynecology rounds out the comprehensive care provided in the Affinity Medical Home model. In addition to comprehensive obstetric and gynecological care, Weiss has special interest in menopause management and ten years of experience in performing gynecological surgery.
Weiss received his medical degree from the Medical College of Wisconsin in Milwaukee and completed his residency at the University of Wisconsin Hospital and Clinics. He is board-certified in obstetrics and gynecology and is a fellow of the American College of Obstetricians and Gynecologists.
Weiss sees patients at the Madison Street Clinic Appleton (1531 S. Madison St., 3rd Floor) Mondays, Tuesday, Thursdays and Fridays.

For the patient, Affinity Medical Home provides access and care in a comprehensive team environment. The personalized care teams at both Affinity Medical Homes (Fieldcrest Drive in Kaukauna and Koeller Street in Oshkosh) consist of one or more physicians partnered with a nurse practitioner or physician assistant, a nurse specialist who will coordinate disease management, a behavioral health resource, a patient service representative and health care associate such as a LPN (licensed practical nurse), MA (medical assistant) or CMA (certified medical assistant).
