Where can I get an application?

What is a "CVO"?
A Credentials Verification Office (CVO) receives and processes practitioner applications in accordance with NCQA standards. When an application is received, elements such as license, education and training, board certification, malpractice, sanctions, previous employment, gaps in training/work, references, etc. are verified at the source to ensure accuracy and viability. In addition, ongoing monitoring of practitioners' files takes place to keep practitioner licensure, certification and health information up-to-date.

How long does the application/credentialing process take?
The application process can take anywhere from 30-90 days. The speed of the process depends upon the completeness of the information submitted by the applicant, the response time of the references, and the timing of the Credentials Committee, Medical Executive Committee, and Professional Activities Committee meetings once the application is considered complete.

When does the Credentials Committee meet?
The Credentials Committee meets the fourth Tuesday evening of every month.

At which facilities may I practice?
Successful applicants to the Affinity Health System Medical Staff will be awarded staff membership at Calumet Medical Center, Midway Surgery Center, Mercy Medical Center and St. Elizabeth Hospital. Privileges approved will be valid for all four facilities, as appropriate to each facility.

How do I change my staff status?
You can fill out a Request for Change in Privileges form form if you desire a change in status or specialty, or wish to resign from the staff.

How do I change (add/delete) my privileges?
You can fill out a Request for Additional Privileges or Request for Change in Privileges form if you desire to add a new privilege or relinquish a seldom used privilege.

How do I go on Leave of Absence?
Any practitioner who will be absent for one month or more must apply for leave of absence status from the medical staff in accordance with medical staff policy.  A practitioner who is ready to return from leave of absence must submit a written request for reinstatement to the Executive Committee and supply such other information as may be requested before the request is processed in accordance with medical staff policy.

Who needs to be notified of a change (address, phone, name, etc.)?
Changes in demographic information should be forwarded to the Affinity Health System CVO (see Medical Staff Services Department Directory).

Where can I get a copy of my CME attendance?
CME programs sponsored by Affinity Health System are tracked by the hospital Medical Staff coordinators. You can contact Carole Carow at Mercy Medical Center (ccarow@affinityhealth.org) or Lisa Sorenson at St. Elizabeth Hospital (lsorenso@affinityhealth.org) for a printout of programs you have attended at Affinity Health System.
