How to Make a Gift
Cash: Make gifts by check or credit card in person or by mail to: St. Elizabeth Hospital Community Foundation, 1506 S. Oneida St., Appleton, WI 54915-1397. You may also call (920) 831-1475.
Securities: The foundation office and our brokerage firm, Robert W. Baird in Appleton, can assist you. Gifts of paper stock or electronic transfer can be made.
Electronic stock gifts: You may make gifts by electronic transfer of stock through our account at Robert W. Baird & Company in Appleton, Wisconsin. Please contact Tom Gottsacker at Robert W. Baird at (920) 738-9181 or 1-888-792-0466 ext. 3749. Transfer stock to DTC number 0547 and instruct to credit the St. Elizabeth Hospital Community Foundation Account # 79610470. It’s helpful if you can notify the foundation of your plans by calling us at (920) 831-1475.
Qualified charitable plan: Name the foundation as beneficiary of a qualified charitable plan (IRA, Tax-Deferred Annuity, 403B, etc.). A simple beneficiary form or change of beneficiary form is required. Contact the foundation office at (920) 831-1475 with any questions.
Gifts that produce income for the donor: Gifts should be discussed directly with the foundation office by calling (920) 831-1475. A variety of options are available, including charitable gift annuities and charitable trusts.
Real estate or life insurance: Gifts of real estate or life insurance require information and documentation. Please gather documents related to the type of gift you are considering and contact the foundation office at (920) 831-1475.
Bequest provision: The foundation can provide you or your advisor with the legal name and preferred language for including the foundation as a beneficiary of your will or trust. Gifts in-kind: The foundation welcomes non-cash contributions of items that directly benefit patients, as long as the department accepting the items determines the contribution is appropriate. Please contact the foundation to ask about making gifts in-kind. When accepting an in-kind contribution, the foundation will complete its section of IRS Form 8283, which is used by those who itemize on their income tax returns. This form allows the donor to claim tax deductions for non-cash charitable gifts. It is the donor’s responsibility to indicate the value of the gift and to fill out various sections of the form, depending on that value.
